LEARNING TAKES PLACE IN QUALITY ASSURANCE
The Canadian Safety Management System (SMS) is designed to continuously improve aviation safety by identifying hazards, managing risk, monitoring performance, and learning from operational experience. Among the six SMS components, Training and Quality Assurance perform complementary but distinctly different functions. Although they are often discussed together, they serve different purposes within an effective safety management system. Training provides individuals with the knowledge, skills, and procedures required to perform their duties safely, while Quality Assurance determines whether those duties are being performed effectively and whether improvements are required. In practical terms, training prepares people to do the job, while Quality Assurance enables both individuals and organizations to learn how to do the job better.
Training is a planned and structured activity. It introduces personnel to regulations, company policies, operating procedures, hazard reporting systems, emergency response plans, and safe work practices. During initial commercial pilot training, pilots learn the Canadian Aviation Regulations, aircraft systems, standard operating procedures, decision-making processes, and the responsibilities associated with operating within an SMS. They also learn how to recognize hazards, report occurrences, manage operational risks, and understand the objectives of the organization's Safety Management System. The purpose of training is to establish a common foundation of knowledge so that every employee begins with the same understanding of expectations and responsibilities.However, completing training does not automatically mean that learning has occurred. A pilot may successfully complete examinations, pass flight tests, or attend mandatory classroom sessions without fully understanding how that knowledge should be applied under changing operational conditions. Training measures whether information has been delivered, while learning measures whether that information has been understood, retained, applied, evaluated, and continuously improved. This distinction is essential because aviation operates in an environment where conditions, technologies, regulations, weather, airports, aircraft, and operational risks continually change. Learning takes place in Quality Assurance.
Learning develops through experience. It occurs when pilots and organizations compare expected performance with actual performance, identify differences, determine why they occurred, and implement improvements. This continuous cycle of observation, evaluation, correction, and improvement is the primary purpose of the Quality Assurance component of the Canadian Safety Management System.
Quality Assurance is often misunderstood as simply conducting audits or inspections. While audits, evaluations, observations, and monitoring activities are important tools, they are not the ultimate objective. Their purpose is to generate reliable information that allows the organization to learn. Quality Assurance determines whether policies, procedures, training programs, and operational practices are producing the intended safety outcomes. When differences are identified, the organization investigates the reasons, develops corrective actions, evaluates their effectiveness, and incorporates the lessons learned into future operations.
This continuous learning process distinguishes Quality Assurance from Training. Training answers the question, "What should people know before performing the task?" Quality Assurance answers the question, "What have we learned after performing the task?" Both questions are essential, but they occur at different stages of organizational performance.For commercial pilots, this distinction is particularly important. Every flight provides opportunities to learn. Weather conditions differ, airports present unique challenges, aircraft systems behave differently, passengers introduce varying operational demands, and human performance changes with fatigue, workload, and environmental conditions. Training cannot anticipate every possible situation a pilot will encounter throughout a career. Instead, training provides the foundation that enables pilots to safely manage situations they have never previously experienced. Quality Assurance then captures those experiences, analyzes them, and determines whether improvements should be made to procedures, training programs, documentation, or operational practices.
Hazard reporting and occurrence reporting illustrate this relationship. Pilots are responsible for reporting hazards, incidents, operational concerns, procedural difficulties, and safety observations encountered during normal operations. These reports are not simply records of what happened; they become learning opportunities for the entire organization. Quality Assurance reviews the reports, identifies trends, evaluates risk controls, determines whether existing procedures remain effective, and recommends improvements where necessary. The knowledge gained from these reports becomes organizational learning that benefits current and future employees.
Training alone cannot create continuous improvement because it is based primarily on existing knowledge. Quality Assurance expands that knowledge by examining actual operational performance. When recurring hazards are identified, corrective actions are implemented, procedures are revised, and future training is updated to reflect new understanding. In this way, Quality Assurance continuously strengthens the Training component by ensuring that training materials remain accurate, relevant, and based on operational experience rather than assumptions.The relationship between Training and Quality Assurance components is therefore cyclical rather than independent. Training prepares employees to operate safely. Operational experience generates information. Quality Assurance analyzes that information and produces lessons learned. Those lessons are incorporated into revised procedures, documentation, and updated training programs. Employees then receive improved training based on real operational experience, completing a continuous cycle of organizational learning and improvement.
This relationship also reflects one of the fundamental principles of modern Safety Management Systems: safety is not achieved simply by complying with procedures but by continually evaluating whether those procedures remain effective. Regulations establish minimum requirements, but Quality Assurance determines whether the organization's actual performance meets or exceeds those expectations. Whenever gaps are identified, learning occurs, and improvements follow.
For pilots, learning extends beyond technical flying skills. It includes decision making, communication, crew coordination, workload management, situational awareness, hazard recognition, and operational judgment. Many of these skills continue to develop throughout an entire career because they are refined through reflection, feedback, investigation, and practical experience rather than classroom instruction alone. Quality Assurance provides the structured process that captures these experiences and transforms them into measurable safety improvements.Organizational learning also supports a proactive safety culture. Rather than waiting for accidents to reveal weaknesses, Quality Assurance encourages organizations to learn from routine operations, minor occurrences, hazard reports, audits, observations, and performance monitoring. This proactive approach identifies opportunities for improvement before serious consequences occur. Employees become active participants in improving safety instead of passive recipients of training.
Training and Quality Assurance are not viewed as competing components. Training establishes competence, while Quality Assurance develops capability. Training teaches employees what is currently known, whereas Quality Assurance discovers what still needs to be learned. Training transfers knowledge from the organization to the employee; Quality Assurance transfers knowledge from operational experience back to the organization. Together they create a continuous cycle of improvement that strengthens both individual performance and organizational safety.
Within the Canadian Safety Management System, this distinction is fundamental. Training provides pilots with the knowledge, skills, and responsibilities necessary to begin operating safely within Canada's aviation system. Quality Assurance ensures that those operations are continuously evaluated, that hazards and occurrences become opportunities for learning, and that the lessons gained from everyday operations improve future procedures, documentation, risk management, and training. The result is a Safety Management System that does more than teach people how to work safely, it enables the entire organization to continually learn how to become safer.
